What is Liva Health Insurance?

Liva health insurance includes a range of plans. These policies are 

  • Offered by Liva
  • Underwritten by National Life & General Insurance Company (NLGIC)
  • Managed by Inayah as the third-party administrator (TPA)

Inayah insurance plans are specifically structured to meet the medical insurance requirements of individuals and families in the UAE, including both residents and dependents. 

Liva offers a tiered structure of insurance coverage through plans such as Platinum, Gold, Silver, Bronze and Chrome, each with defined benefits. These plans include —

  1. Worldwide coverage for treatment (up to 90 days per policy year outside UAE)
  2. Annual medical coverage limits ranging from AED 150,000 to AED 1,000,000
  3. In-patient and out-patient services, including hospitalisation, surgeries, and specialist consultations
  4. Maternity coverage (inpatient and outpatient) with coinsurance applied
  5. Access to a large medical network including hospitals, clinics, and pharmacies across the UAE
  6. Coverage for chronic and pre-existing conditions up to AED 150,000 per member per year (subject to policy terms)

Liva medical insurance plans are flexible. They offer optional co-payment levels (0%, 10%, or 20%) and comply with DHA and other local health authority standards. 

Note: All members need to declare existing conditions and undergo a medical application process if required.

Liva Platinum Plan (Highest coverage)

  • Annual Limit: AED 1,000,000
  • Network Access: Access to top-tier hospitals and specialists across the UAE
  • Territory: Worldwide cover (including emergency care abroad up to 90 days/year)
  • Room Type: Private room in hospital
  • Maternity Coverage (In-patient): Up to AED 10,000 with 10% coinsurance
  • Maternity Coverage (Out-patient): With 10% coinsurance
  • Physiotherapy: 15 sessions/year (10% co-pay)
  • Parent Accommodation for child (under 18): AED 100/day
  • Organ Transplant & Cancer Treatment: Covered
  • Vision, Hearing Aids, Vaccinations: Included as per DHA policy

Best for: Families, corporate executives, or individuals needing comprehensive global and local coverage.

Liva Gold Plan (High coverage with slight reductions)

  • Annual Limit: AED 750,000
  • Network: Gold-tier hospitals and clinics
  • Room Type: Private
  • Maternity (In-patient): Same as Platinum — AED 10,000 limit
  • Maternity (Out-patient): Covered with 10% coinsurance 
  • Diagnostics & Medicines: Covered with optional co-pays (0%, 10%, 20%)
  • Vision, cancer treatment, child vaccinations, and organ transplant: Covered
  • Physiotherapy & Parent Stay: Same as Platinum

Difference from Platinum: Slightly lower annual limit and more limited network.

Liva Silver Plan (Balanced mid-tier option)

  • Annual Limit: AED 500,000
  • Network: Silver-tier medical centers and clinics
  • Room Type: Private
  • Chronic Conditions: Covered up to AED 150,000
  • Maternity Benefits: Identical to Platinum & Gold (but within Silver network)
  • Diagnostics & Medicines: Covered (co-pays apply)
  • Ambulance, vaccinations, and preventive services: Included

Difference from Gold: Lower annual limit and smaller network size. It’s good for those wanting decent coverage at a moderate price.

Liva Bronze Plan (Essential coverage only)

  • Annual Limit: AED 250,000
  • Network: Bronze-tier hospitals and clinics
  • Room Type: Private
  • Chronic Conditions: Covered up to AED 150,000
  • Maternity (In-patient): Up to AED 10,000 with 10% coinsurance
  • Maternity (Out-patient): Covered with 10% coinsurance 
  • Out-patient Treatment: 20% co-pay applies
  • Vaccinations, organ transplant, and ambulance: Covered with plan limits

Key Difference: Basic network and lower annual limit. This policy is suitable for visa-compliance or minimal health needs.

Liva Chrome Plan (Entry-level / clinic-restricted)

  • Annual Limit: AED 150,000
  • Network: Chrome (limited to outpatient clinics only)
  • Room Type: Private
  • OPD Restricted: No hospital outpatient coverage — clinics only
  • Maternity: Same as Bronze, but in limited network
  • Diagnostics & Medicines: Covered with co-pay
  • Chronic/Pre-existing Conditions: AED 150,000 max
  • Physiotherapy: 15 sessions/year (10% co-pay)

Key Difference: It’s the lowest-priced plan, designed for basic medical needs through clinics. Note that it doesn’t provide access to hospitals for outpatient services.

📝 Note: All plans cover pre-existing and chronic conditions (subject to limits), repatriation of mortal remains (AED 7,500), and DHA-compliant preventive care. However, network access, annual limits, and maternity benefits vary by plan tier.

In simple terms, you won’t find any standalone Inayah insurance. Rather, Inayah is the operational partner that ensures your Liva insurance policy works smoothly — whether it’s finding a hospital, getting a pre-approval, or settling a claim.

Inclusions & Exclusions of Liva Health Insurance

✅ What’s Included (Inclusions)

With Liva insurance and Inayah TPA, you get coverage for a wide range of medical services based on the plan you choose. Here are the common inclusions across most plans — 

  1. InPatient Care: Hospitalisation, surgeries, doctor fees, anesthetist charges, and private room accommodation
  2. OutPatient Care: Consultations, specialist visits, diagnostics (X-ray, MRI, ultrasound, CT), and prescribed medications
  3. Emergency Ambulance Services: Covered for transportation to and from hospitals within the UAE
  4. Maternity Services: In-patient and out-patient maternity care included (limits vary by plan)
  5. Pre-existing & Chronic Conditions: Covered up to AED 150,000 per member, per year (after declaration and underwriting)
  6. Physiotherapy: Up to 15 sessions per year with co-pay (10%)
  7. Child Vaccinations: Covered as per DHA guidelines
  8. Cancer Treatment & Organ Transplant: Covered under all major plans

❌ What’s Not Covered (Exclusions)

While Inayah TPA is quite comprehensive, there are certain treatments and conditions that are not covered unless specifically included in your selected policy —

  1. Undeclared pre-existing conditions or chronic illnesses
  2. Pregnancy not disclosed at the time of application
  3. Infertility treatment, IVF, or related medications
  4. Cosmetic or elective surgeries not medically necessary
  5. Alternative therapies (unless covered in policy)
  6. Experimental or non-approved treatments
  7. Non-DHA-approved vitamins, supplements, or health products
  8. Self-inflicted injuries, drug, or alcohol-related conditions
What documents are needed to apply for Liva Health Insurance?

To apply, you’ll need —

  • Passport copy with valid UAE visa page
  • Emirates ID copy (both sides)
  • Certificate of Continuity (if applicable)
  • Completed and signed Medical Application Form (MAF)
Can I choose my own co-payment level in the Liva health insurance plan?

Yes. Depending on your selected plan or employer policy, co-payments for doctor visits and medicines can be set at 0%, 10% or 20%. 

Is the Liva medical form valid forever?

No, the Medical Application Form (MAF) is valid for 30 days only from the date it’s completed and signed by the applicant.

What if I’m already pregnant when applying for Liva Health Insurance?

If you’re pregnant at the time of applying, you must declare it in the Medical Application Form. Failing to do so can lead to claim rejection or policy cancellation.

How are pre-existing or chronic conditions handled?

These conditions are generally covered up to AED 150,000 per year, although they must be declared in the application form. Coverage starts after underwriting approval and may have waiting periods based on DHA guidelines.

Can children be covered under the same Liva health insurance policy?

Yes. You can include your spouse and children as dependents under the same health insurance policy. Their full details — including relationship, age, nationality, and medical information — must be provided in the form.
