HAYAH Claim / Reimbursement Process

Unlike direct billing, reimbursement claims are made if you avail of medical treatment at a facility outside the network of your health insurance provider. To submit your HAYAH claim/reimbursement form, keep the following steps in mind - 

  • Fill out the reimbursement form: Get the form from the provider and fill it out thoroughly.
  • Attach relevant documents: After completing the form, attach the relevant documents to your claim reimbursement form. This can include your medical card, reports, expenditure records, and more. 
  • Submit the form: After filling out the form and attaching the specified documents, you can submit the claim form using the relevant TPA mobile application.

Step 5 - Settlement
