Age

Address Proof

  • A copy of the applicant’s Emirates ID.
  • A copy of the applicant’s passport.

  • A signed copy of the credit bureau consent form. This form signifies that the American Express credit card applicant allows Amex to share basic personal information to conduct a credit history check. The form can be found online.
